Nearly 1 million Americans could receive $500 ACA refund checks in October 2026. The payments target eligible HealthCare.gov users in 30 states who paid premiums without assistance.
Trump $500 ACA Refund Checks: Nearly 1 million Americans are now set to receive a $500 refund from the federal government over Affordable Care Act health insurance fees. The U.S. Treasury has started sending the checks to more than 950,000 people across 30 states. The White House had announced the refunds in September and said payments would begin in October 2026.
The payment is being called the Working Families Obamacare Refunds. According to the Trump administration, the money comes from excess user fees collected from insurers using the federal HealthCare.gov marketplace. The administration says these fees were passed to consumers through their insurance premiums and that some money remained after the marketplace’s operating costs were covered.

Each eligible person will get $500. With more than 950,000 recipients, the total value of the refunds is around $500 million.
The payment isn’t for everyone who has an Affordable Care Act plan. The main group being targeted is people who bought their insurance through HealthCare.gov and didn’t receive federal premium assistance. That means they paid the full cost of their marketplace premiums without a premium tax credit.
The refunds are limited to people in 30 states that use the federal marketplace. They are:
Alabama, Alaska, Arizona, Arkansas, Delaware, Florida, Hawaii, Indiana, Iowa, Kansas, Louisiana, Michigan, Mississippi, Missouri, Montana, Nebraska, New Hampshire, North Carolina, North Dakota, Ohio, Oklahoma, Oregon, South Carolina, South Dakota, Tennessee, Texas, Utah, West Virginia, Wisconsin and Wyoming.
People who live in states that run their own ACA exchanges won’t be part of this refund program. The government is using previously collected marketplace funds for the payments.
For 2026, CMS set the user fee for insurers on the federally facilitated marketplace at 2.5% of monthly premiums. These fees help pay for the operation of the federal exchange.

The Treasury Department has started sending refunds. The first checks began being issued on September 30, with eligible recipients receiving the money along with a letter signed by Trump.
People who qualify don’t need to submit a new application for the refund. The program has already identified recipients, and the payment is being sent directly to them.
The administration has described the payment as a return of money collected through marketplace fees. In the letter being sent with the checks, Trump said v“You have paid into this flawed System, and now you are finally getting something back.”

He also wrote, “That money belongs to hard-working Americans, not the Government, and now I’m returning it to you!”
The refunds are arriving at a time when ACA costs have changed for many households. Enhanced premium tax credits expired at the end of 2025, and KFF says average monthly premium payments after tax credits were $178 during 2026 open enrollment.
The $500 payment is a one-time refund. It doesn’t mean that ACA premiums or future insurance costs will automatically fall by the same amount.
