• At the time of death, the veteran was awaiting payment on an outstanding original or reopened claim for VA pension or benefits.
• The veteran passed away on October 9, 1996, or later while a resident of a state nursing home with VA approval.

• Social Security number or VA file number
• Date and place of birth
• Date and place of death
• Military service history
• Date of burial
• Final resting place

Your details, including your date of birth, mailing address, Social Security number, and contact details, will also be required.

Additionally, the following files must be submitted:

• Death certificate (including the cause of death)
• Itemized receipt for transportation costs (if applicable)
• The veteran’s DD214 or other separation documents
