VA Burial Benefits Form: The Department of Veterans Affairs (VA) offers financial assistance to help with burial and funeral expenses in the event of a veteran’s passing.
The VA burial allowance is the name given to this payment.
The veteran must have been released from service under circumstances other than dishonourable and satisfy at least one of the following requirements to be eligible for VA burial benefits:

- The veteran’s disability from service caused his death.
- At the time of death, the veteran was either receiving or eligible to receive a VA pension or compensation.
- The veteran passed away either while receiving VA-contracted treatment or while being admitted to a VA hospital.
- The veteran passed away while traveling for an evaluation, care, or treatment at VA expense and with the appropriate authorization.
- At the time of death, the veteran was awaiting payment on an outstanding original or reopened claim for VA pension or benefits.
- The veteran passed away on October 9, 1996, or later while a resident of a state nursing home with VA approval.
You will require the following details and paperwork regarding the veteran who has passed away in order to apply for funeral benefits:
- Social Security number or VA file number
- Date and place of birth
- Date and place of death
- Military service history
- Date of burial
- Final resting place
Your details, including your date of birth, mailing address, Social Security number, and contact details, will also be required.
Additionally, the following files must be submitted:
- Death certificate (including the cause of death)
- Itemized receipt for transportation costs (if applicable)
- The veteran’s DD214 or other separation documents
